Transitions with Sanarte

Navigating the Step-Down from Residential or PHP/IOP Successfully Through Team-Based Outpatient Care

Gabe Aranovich

“When kids are stepping down from residential, PHP, or IOP, that transition can be really tough. What I love about Sanarte is that it’s set up to make that process smoother. The care is team-based, really comprehensive, and because it’s delivered using a concierge model, families get the kind of support that actually helps them keep moving forward — not just maintain progress, but build on it.”

Helping Families Navigate Transitions in Care

Transitions from residential, Partial Hospitalization (PHP), or Intensive Outpatient (IOP) care can be difficult to navigate. Sanarte’s team-based, concierge model provides consistent, expert support to help patients maintain progress and adapt at home.

Our team-based, concierge approach provides expert support through every step of the transition. The members of the Sanarte Care Team, typically consisting of a psychiatrist, an individual therapist, a family therapist, and a care coordinator work closely together with all external providers to ensure care remains connected, coordinated, and responsive during the change.

We help families reestablish routines, adjust to new roles, and respond to challenges as they arise. Our team meets regularly to review progress and update the care plan, so nothing is left unaddressed. By staying closely involved and highly responsive, we make it easier for families to carry the progress made in residential or PHP/IOP treatment into everyday life.

For families and referrers looking for thoughtful, structured support after a higher level of care, Sanarte provides a reliable path forward.

We carefully match each client with a team of specialists best suited to their unique needs.

Why is family therapy part of every treatment plan?

Behavioral health experts widely agree that involving the whole family can lead to better outcomes, faster healing, and longer-lasting change. Children and teens live and function in the context of their families. Therefore, they do better when their whole family does better.

For some challenges, such as eating disorders or substance abuse, research shows that family therapy is the most effective treatment. Whether a child or teen is facing anxiety, depression, identity struggles, or school stress, the family is a critical part of the solution. Family therapy supports the parents in being the best they can be, and creates a structured, supportive space to improve communication, rebuild trust, and help everyone better understand each other.

What if a family member is hesitant about family therapy?

Our clients often come to us with at least one member who is resistant to engaging in therapy. Our team specializes in engagement-first approaches designed to ease that resistance. A few key points:

  • While family therapy can seem challenging, recovery tends to be much more effective when the whole family system is involved, and when caregivers are supported at being at their best.
  • Our family therapists use evidence-based methods to build trust and motivation.
  • We encourage family members to participate at their own pace.
  • We focus on practical, immediate benefits, helping even reluctant participants see the value in treatment.
  • We create a space where every family member feels comfortable, heard, and supported.
What if the client already has a therapist or psychiatrist that they are happy with?

We fully support continuity of care and are committed to collaborating with existing medical and mental health providers to ensure a seamless, integrated experience. Our team is happy to coordinate with current therapists and/or psychiatrists. With that said, most Sanarte clients appreciate the opportunity to gradually transition to Sanarte’s clinical team, allowing for even greater coordination and a truly holistic approach to exceptional care.
